1. Application Form
We offer a wide range of high quality of teaching and extra-curricular activities.
This form must be completed in full. All changes to be initialled or signed by parent/guardian. Completing the form does not necessarily mean that the learner has been accepted. The following documents need to accompany the application form:
- Passport photo of the learner/Paspoort foto van leerder.
- Copy of Immunisation Records / Afskrif van inentingsertifikaat
- Copy of Abridged Birth Certificate / Afskrif van Onverkorte geboortesertifikaat.
- Progress Report from Previous School / Vorderingsverslag van vorige skool.
- Transfer Letter from Previous School / Oorplasingsbrief van vorige skool (needed when application is approved).
- Telephone/e-mail address / Telefoon nommer/e-pos adres.
- Id of both parents/Id van beide ouers.
- Id of Payee if different from Father or Mother’s/Id van betaler indien verskillend van Ma en Pa.
- Certified copy of death certificate if one of the parents are deceased/ Beëdigde afskrif van dood serifikaat indien ouer oorlede is.
- Proof of address e.g. municipality account or bank statement (No counsellors letters allowed)/Bewys van adres (geen raadslid briewe ).
2. Aftercare Application Form

Please note that 1 month’s notice to be given prior to cancelling aftercare, failure to do will result in parents being liable to pay a penalty of a month’s aftercare fees. No cancellations will be entertained from the 1 September for the current year. Aftercare is an additional service offered by the school and NO exemption application can be made in this regard. Aftercare runs from January till November every year.
